Consumer Information

In accordance with Texas law (House Bill 4224 and Section 181.105 of the Texas Health and Safety Code), the following information is provided to help consumers understand their rights and available resources.

You have the right to request access to your health records. To request a copy of your records, please:

  • Submit a written request by email, secure portal message, or in person
  • Include your full name, date of birth, and the specific records requested

Requests are processed within 15 days of receipt in accordance with Texas law and applicable regulations, including allowable fees. Per Texas regulations, the following fees apply for copies of medical records:

Paper Records: $25.00 for the first 20 pages / $0.50 per page after the first 20
Electronic Records (if stored electronically): $25.00 for up to 500 pages / $50.00 for more than 500 pages

If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.

Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.
