Medication Evaluation & Management

Our board-certified Psychiatric Mental Health Nurse Practitioner, Amy Cremer, APRN, PMHNP-BC, provides specialized medication evaluation and management services for children (ages 6+), adolescents, and adults.

Amy brings together medical expertise and a deeply compassionate approach—because meds aren’t just about prescriptions; they’re about people. Whether it’s your first time exploring medication or you’ve tried things before that didn’t quite work, Amy takes the time to listen, educate, and partner with you in finding what actually helps you feel better.

Our Approach to Medication Management

We believe medication is a tool—often a powerful one—but rarely the entire solution. Our approach to psychiatric medication is thoughtful and conservative, focusing on:

  • Collaboration: Your input is essential in determining the right treatment approach. We’ll explain options clearly so you can make informed decisions about your care.
  • Individualization: Each person responds differently to medications. We’ll carefully consider your unique biochemistry, lifestyle, and preferences when making recommendations.

Amy provides medication management for a range of mental health conditions, with particular expertise in:

What to Expect at Your Psychiatric Appointments with Us

Initial Psychiatric Evaluation (50-60 minutes)

Your journey begins with a thorough evaluation where we’ll:

  • Discuss your current symptoms and concerns
  • Review your medical and mental health history
  • Explore family history that may be relevant to your care
  • Examine how your symptoms impact your daily functioning
  • Consider any previous treatments and their effectiveness
  • Discuss your goals and what wellness looks like for you

Following this comprehensive assessment, we’ll work with you to develop a personalized treatment plan that may include medication recommendations, therapeutic approaches, and lifestyle modifications.

Medication Management Follow-Up Sessions (25-30 minutes)

These regular sessions allow us to:

  • Provide education about your condition and treatment options.
  • Monitor your response to medications
  • Address any side effects or concerns
  • Make adjustments to your treatment plan as needed
  • Track your progress toward your wellness goals

That’s a completely valid concern—and one I hear often. My approach is always rooted in education, collaboration, and respect for your unique needs and experiences. I never believe in a one-size-fits-all solution when it comes to mental health, and I’m here to help you feel informed and empowered every step of the way.

I provide credible, easy-to-understand resources about medications—including side effects, what to expect, and how they work—so you’re never left in the dark. If you have reservations, we’ll take time to talk through them together. If a particular medication doesn’t feel right to you, we’ll explore alternatives. There are many effective options, and my goal is to find the one that fits best for you.

If you have a history of dependency or are worried about that, I absolutely take that seriously. There are many effective medications that do not carry a risk of dependence, and we’ll be thoughtful about what’s safest and most appropriate for your situation.

Ultimately, I believe that listening closely to your concerns and making decisions together is key. My job isn’t just to prescribe—it’s to partner with you in finding a treatment plan that feels right and supports your long-term wellness.

Every person is unique, and so is their mental health journey. When we meet, we’ll take the time to talk through your symptoms, history, goals, lifestyle, and any concerns you have. From there, we’ll work side by side to build a treatment plan that feels aligned with your needs—not just clinically, but personally.

There are many medications within each class, and part of my role is to use my clinical knowledge to guide you toward options that are safe, effective, and appropriate for your individual health profile. For example, certain medications may be a better fit for someone with specific medical conditions or a history of certain side effects.

But just as important as clinical expertise is your voice in the process. I’ll always make sure you feel informed, heard, and fully comfortable with the plan before moving forward. Treatment isn’t something I prescribe to you—it’s something we create with you.

Your first appointment is all about getting to know you. I’ll start with a thorough assessment so I can fully understand your symptoms, history, and goals. From there, I may offer a diagnosis based on what we discuss, and we’ll work together to create a treatment plan that’s personalized, supportive, and aligned with your needs.

You can expect to be met by a warm, nonjudgmental, and attentive provider—someone who truly listens and creates a safe space for you to open up. This is your time, and I’m here to walk alongside you as you work toward greater clarity, balance, and wellness.
